Date: October 3, 2009
Reason for Visit: Bilateral front limb “head bob” and occasionally tripping on front limbs. Bilateral Patellar luxations.
Today’s notes: Dr. Kopman and other interested parties.
Emmy definitely has bilateral lateral patellar luxations of equal magnitude(2/4). There is no evidence for cartilage damage or pain associated with these luxations yet. Emmy also has a fairly “choppy” forelimb gait with mild pain at the extreme of elbow and/or shoulder extension on both sides. Radiograms of the forelimbs do not support significant elbow incongruity. They do show what appears to be some proximal humeral deformation. The humeral articular surface appears to be rocked caudally on these radiograms. Further diagnostics would be required to better characterize this. This type of deformity could explain the forelimb gait.

The patellar luxations will reduce strength of stifle extension and potentially damage cartilage and/or strain soft tissues and result in pain. These degenerative changes may never become significant enough to result in surgery. However once cartilage damage occurs it is irreversible. Based on these considerations the owners have elected to monitor the luxations carefully. For any pain or progression of pathology.
Treatment of the humeral deformity (if real) could require humeral osteotomy and the result would not be guaranteed to smooth out the gait. Therefore conservative management has been elected at this time.

Patellar luxations are a developmental abnormally that are highly likely to have genetic origins and therefore be heritable. Further, the abnormality in the shoulders falls into the same category.
This information should be considered when deciding on breeding patients with these kinds of developmental abnormalities.
